Thyrohyoid Approximation
Pitch is only half of what makes a voice sound feminine. The other half is resonance: how bright or dark the sound is as it travels up through the throat and mouth. A shorter, smaller space above the vocal folds gives a brighter sound, which is why voice therapists teach people to lift the larynx when they speak. Thyrohyoid approximation, usually shortened to TA, does that lift with stitches.
Through a small cut high on the neck, the surgeon draws the thyroid cartilage up towards the hyoid bone, the small U-shaped bone under the jaw. The voice box then sits higher for good, and the throat above the folds is shorter. Most surgeons who offer TA do it in the same operation as a glottoplasty, which raises the pitch, so the two changes arrive together.
TA changes the colour of the voice, not the note it sits on, and it does its best work when pitch is being raised at the same time. For anyone who has fought to keep their resonance bright on a tired day, that is the appeal. The glottoplasty and training set the pitch, and TA helps that brightness hold without constant effort. It is specialist surgery, done by a smaller group of voice surgeons than the main pitch operations.

Who it’s for
The usual candidate is planning a glottoplasty and has worked hard on resonance in training. They can brighten their voice when they concentrate, and it darkens again when they are tired or relaxed. Their surgeon thinks a raised pitch on its own would leave a voice that still sounds heavier than they want.
TA also suits some people whose glottoplasty raised their pitch well but who still feel their voice sounds dark. Resonance can be worked on after a glottoplasty, and a surgeon may suggest TA if training has stalled. Some people also like the way it draws the Adam's apple up and back under the jaw, which makes it less visible.
If you have not trained yet, start there. Training changes resonance a long way for most people, and the surgeon needs to hear what yours can do before deciding whether TA would help. If you have trouble swallowing, or reflux that is not under control, say so early, because TA changes how the voice box moves when you swallow. Singers should raise it too.
You do not need to have been on hormones, since oestrogen does nothing to the larynx. Surgeons want you off nicotine for several weeks either side, and any reflux treated first.

At the assessment
Ask your therapist to record you speaking with your larynx lifted as high as you can comfortably hold it. Play it to the surgeon, because it is the closest thing you have to hearing what TA might add.
How it’s done
The operation copies something your body does when you swallow or sing a high note. Muscles in the neck pull the voice box up towards the hyoid bone, and the space above the folds shortens. TA holds it there permanently.
The incision is short and horizontal, high on the neck in a natural crease above the Adam's apple. The thin muscles in front of the voice box are eased apart. That exposes the top of the thyroid cartilage and the hyoid bone above it. Some surgeons trim the membrane and soft tissue between the two so they can be brought close together.
Strong stitches then pass around the hyoid bone and through the upper edge of the thyroid cartilage. Drawing them together lifts the voice box and tucks it up behind the hyoid. Some surgeons shave a little of the cartilage's upper edge first, which helps the two fit together and can reduce the prominence at the same time. The muscles and skin are closed, sometimes over a small drain.
When TA is combined with a glottoplasty, the glottoplasty comes first, through the mouth with a camera. TA follows through the neck under the same anaesthetic. Nothing in TA itself enters the airway.

The week before
Stock up on soft foods and drinks you enjoy: yoghurt, soup, smoothies, eggs. Swallowing feels strange for the first days, and having easy things ready makes eating well much less of an effort.
Thyrohyoid approximation recovery
Swallowing is the part most people notice. With the voice box held higher, it moves differently when you swallow. For the first week or two, food and drink can feel as if they catch. Soft foods and small sips help, and most people adapt within a few weeks. The neck is sore and swollen at the front, and turning or tilting the head feels stiff.
Voice rest follows the plan for whatever was done with it. After a glottoplasty that means about a week of complete silence. After TA alone, surgeons usually ask for a shorter rest. Then comes a slow, rationed return, paced by your surgeon and therapist.
After that, recovery is retraining. Your voice now sits in a slightly different throat, and therapy helps you find the new resonance without pushing. Most of the people I have seen get the best result from TA kept up their therapy for months afterwards. Your surgeon's protocol wins wherever it differs from the timeline below.
| Day 0 | Throat and neck feel full and tight. Swallowing is awkward. Silence starts now if a glottoplasty was done. |
|---|---|
| Days 1–7 | Home after a night or two. Soft foods and sips. Neck swelling at its worst, then easing. |
| Weeks 2–3 | Swallowing easier. Short, quiet speech on your surgeon's schedule. Desk work realistic if it doesn't need calls. |
| Weeks 4–8 | Talking time builds. Therapy restarts, working on easy use of the new resonance. |
| Months 3–6 | Swallowing feels normal for most people. Scar softening. Voice steadier and less tiring. |
| Months 9–12 | Resonance and pitch have settled. The fair point to judge the result. |

In the first two weeks
Sit upright to eat and drink, take small mouthfuls, and tuck your chin slightly as you swallow. It is how the throat copes best with the new position while it adapts.

Thyrohyoid approximation scars
One horizontal line, usually a few centimetres, high on the neck in the crease between the chin and the Adam's apple. It sits higher than the scar from a tracheal shave or a cricothyroid approximation, and is often hidden in the shadow of the jaw. It is pink and firm for a few months, then fades to a pale line for most people. On some necks it stays raised or widens, and darker skin can heal darker along the line.
Risks and complications
In the first weeks the risks are those of any short operation on the front of the neck. A bleed or infection in the wound, fluid under the scar, or swelling around the airway that makes breathing feel tight for a day or two. That is one reason surgeons keep you in overnight.
Swallowing is the risk specific to TA. Nearly everyone has some difficulty at first, and a small number have trouble for longer, including food or drink going down the wrong way. For most it settles as the throat adapts, sometimes with help from a speech therapist who specialises in swallowing. Tell your surgeon at once if you cough or choke when you drink.
Later, the stitches can loosen or cut through, and the voice box can drift back down, taking some of the brighter resonance with it. As with other voice operations, the voice can be hoarse or tire faster for some weeks. Long-term follow-up comes from a smaller number of patients than for glottoplasty, so a surgeon's own results are the best guide.
Thyrohyoid approximation results
A good result, judged at around a year, is a voice that sounds lighter and brighter without effort. It holds when you are tired or not concentrating. Combined with a glottoplasty, the two changes together can make a voice that is heard as feminine far more consistently than pitch alone.
TA does not raise pitch much on its own, and it does not replace training. Intonation, vocal weight and the way you shape words still come from therapy, and they finish what the operation starts. Some people find their lower range is reduced, and singers may notice a change in how the voice feels at the top.
Alternatives to thyrohyoid approximation
Training is the first alternative for everyone, and for many people it brightens resonance enough. Voice feminization training works directly on larynx height, and the habits it builds are the ones TA makes permanent.
If pitch is the main thing you want to change, a Wendler glottoplasty is the core operation. TA is an addition to it, not a substitute. A feminization laryngoplasty makes the voice box itself smaller, which changes both pitch and resonance in one larger operation. I would ask any surgeon who recommends TA why your voice needs it on top of a glottoplasty, and to play you recordings that show the difference.
Wendler Glottoplasty
A Wendler glottoplasty joins the front of the two vocal folds into a small web, leaving a shorter length to vibrate and a higher voice. It is done through the mouth and leaves no visible scar. It gives a lasting lift to the pitch your voice rests at, in exchange for the lowest notes and a little volume.
Best for: someone whose trained voice still falls back to a low pitch under strain
Feminization Laryngoplasty
A feminization laryngoplasty opens the voice box through the front of the neck, narrows the thyroid cartilage and shortens the vocal folds inside it, often with a shave of the Adam's apple. It aims at a larger pitch change than glottoplasty and can reduce the Adam's apple in the same sitting. It involves a neck scar and a longer recovery, and it is done by a small group of specialist surgeons.
Best for: someone who has trained, wants a bigger pitch change than glottoplasty offers, and can reach a surgeon who does many of these
Voice Feminization Training
Resonance, weight, pitch, intonation and speech habits: what feminization training changes, how long it takes and how to tell it is working.
Combining thyrohyoid approximation with other surgery
TA is almost always combined, most often with a glottoplasty under the same anaesthetic. The two do different jobs, one shortening the folds and one shortening the throat above them, and they add together.
It can also be combined with a tracheal shave, since both work on the top of the thyroid cartilage, sometimes through the same incision. If a shave is on your list, tell every surgeon involved before either date is set, because TA moves the cartilage the shave would reshape.
Thyrohyoid approximation cost
TA is usually quoted as part of a glottoplasty package rather than on its own, so ask what it adds to the glottoplasty price. The all-in figure should include the pre-operative scope, the surgeon, the anaesthetist, the hospital nights and the follow-up checks.
Ask what happens if swallowing takes longer to settle, and whether swallowing therapy is included or arranged. Voice therapy afterwards is rarely included and is part of getting a good result.
| Country | Self-pay | Public / insurance |
|---|---|---|
| United States Offered by few surgeons; usually priced as an addition to a glottoplasty | Quoted case by case | Rarely covered; usually treated as cosmetic |
| Thailand Ask whether it is offered with a glottoplasty; plan on about two weeks in country | Quoted case by case | Self-pay only for international patients |
| United Kingdom Rarely offered; most UK patients who have it travel abroad | Quoted case by case | Not funded on the NHS |
Choosing a thyrohyoid approximation surgeon
TA is done by laryngologists with a specialist voice feminization practice, and it is offered by fewer of them than the main pitch operations. Ask how many TAs they do a year, and how many of their patients still had swallowing problems at three months.
Ask to hear recordings from a year after surgery, ideally of glottoplasty alone and glottoplasty with TA, so you can hear what the addition does. A surgeon who can play you both is showing you exactly what they offer. Ask, too, how they work with your voice therapist and who you call if swallowing is difficult once you are home.

At the consultation
Ask what they would expect your voice to sound like with the glottoplasty alone, and what TA adds. A surgeon who can explain the difference in your own voice is one who knows exactly why they are recommending it.
Frequently asked questions
Does thyrohyoid approximation raise pitch?
Only a little. Its main effect is on resonance, making the voice brighter, and pitch is raised by the glottoplasty done with it. See results.
Is swallowing difficult after TA?
Yes, at first, for nearly everyone. Most people adapt within a few weeks. See risks.
Can I have TA without a glottoplasty?
Some surgeons offer it alone after an earlier glottoplasty, but most do the two together. See who it's for.
Will TA make my Adam's apple less visible?
It often does, because the cartilage is drawn up and back under the jaw. If a flatter neck is your main aim, a tracheal shave is the operation for it.
